Registered Nurse - Care Manager

Medacs Healthcare are assisting a disability service and support organisation in Western Sydney find a Care Manager for a permanent position.

The role:
As a Registered Nurse, you will demonstrate an ability to provide safe and quality nursing care to patients, utilising the primary nursing model of care and nursing competency standards. You will undertake comprehensive assessments of each patient to ascertain care needs, regularly evaluate the outcomes of care provided through reviews and assessments, maintaining up to date and accurate documentation in patient records.

Skills & Experience:

• Registered Nurse qualification
• Bachelor's degree in Nursing
• 3 years demonstrated relevant experience in aged care management or a similar role
• Knowledge of the Aged Care Act 2024 and its implications for service delivery
• Division 1 AHPRA registration or eligibility to apply
