Location: North Sydney
Employment Type: Full-time, Contract (8 weeks)
Work Arrangement: Hybrid (3 days work from home/remotely - 2 days in office)

The Role:

Medacs Healthcare are seeking an experienced Care Manager specialising in Home Care (Aged Care) for a full time contract based in North Sydney. This role involves coordinating home care services, managing staff, and ensuring compliance with regulatory requirements, while fostering a supportive and safe environment for clients and their families.

Key Responsibilities:

  • Clinical Oversight: Oversee and ensure the safe assessment, planning, delivery, and evaluation of clinical care in accordance with individual care plans.
  • Compliance and Quality Standards: Ensure all care services comply with the Aged Care Act 2024 and relevant regulations.
  • Staff Management: Recruit, train, and supervise direct care workers, and provide ongoing support and professional development.
  • Supervision and Training: Provide guidance and training to non-clinical care partners, relationship managers, support workers, and internal teams.
  • Client and Family Engagement: Establish and maintain positive relationships with clients and their families.
  • Budget Management: Assist in managing care budgets, ensuring efficient use of resources.
  • Documentation and Reporting: Maintain accurate documentation of care plans, incident reports, and compliance audits.
  • Advocacy: Advocate for client's rights and preferences.
  • Continuous Improvement: Participate in quality improvement initiatives.
  • Quality Assurance and Auditing: Support ongoing audit and compliance of aged care services.
  • Serious Incident Reporting Scheme (SIRS): Ensure compliance with SIRS and train the workforce to respond appropriately to serious incidents.
  • Risk Management and Emergency Response: Develop and evaluate risk management and emergency response plans.

Requirements:

  • Over 3 years of experience in aged care management positions - specialising in Home Care
  • Current AHPRA Registration with no restrictions
  • Bachelor of Nursing Degree
  • Strong knowledge of AN-ACC and strengthening standards in Aged Care
  • Experience in leadership and training
