Registered Nurse Mental Health - Case Management

Your Role
- Lead Case Management: Conduct mental health assessments, design care plans, and walk alongside clients on their recovery journey
- Deliver Community-Based Care: Provide services in homes, clinics, and local community hubs
- Crisis intervention: Offer immediate intervention and stabilization for individuals in distress
- Collaborate Across Disciplines: Partner with psychiatrists, GPs, social workers, and allied health professionals
- Empower Clients: Educate individuals and families, and connect them to long-term support networks
What You Bring
- Tertiary qualifications AND current professional registration with AHPRA
- 2+ years' Experience in Mental Health Case Management
- Excellent communication and rapport-building skills
- Cultural competency and respect for diverse communities
- Willingness to travel and work in regional or remote areas
- Full working rights in Australia
