We are seeking a dedicated Community Nurse to join a supportive healthcare team in South Australia. About the Role
As a Community Nurse, you will provide high-quality nursing care to clients within their homes and community settings. Working collaboratively with a multidisciplinary team, you will support patients to maintain their independence, health, and well being.

Key Responsibilities

• Deliver comprehensive community nursing care and assessments.
• Develop and implement patient-centred care plans.
• Provide wound management, medication administration, and chronic disease support.
• Educate clients and families on healthcare management.
• Maintain accurate clinical documentation.
• Liaise with GPs, allied health professionals, and community services.

Essential Requirements

• Current AHPRA Registration as a Registered Nurse
• Community and Mental Health experience
• Current National Police Check
• Valid Driver's Licence
• Excellent communication and clinical assessment skills
• Ability to work independently and as part of a team
